Abstract

Participants in the Chronic Disease Management Program at UPTD Rawalele Community Health Center, Subang Regency, showed that 67.3% of individuals with diabetes had HbA1c levels above 7%, with an average of 7.4%. The main challenges included high daily glycemic loads, low physical activity after meals, and suboptimal self-monitoring. This community service program aimed to improve knowledge and self-management among patients with type 2 diabetes mellitus and strengthen the capacity of supporting community health workers. The program was conducted from August 22 to September 20, 2026, using a community-based participatory research (CBPR) approach involving 50 patients and 21 community health workers. The interventions included education on glycemic load and the healthy plate concept, physical activity after meals, and daily recording through a website-based logbook. The results showed that community health workers’ knowledge increased from 56.2% to 89.5%, while patients’ knowledge increased from 54.0% to 86.0%. Daily recording adherence reached 88.3%, and the duration of post-meal physical activity increased from 12.6 to 17.9 minutes. Self-monitored blood glucose levels decreased by 11.3 mg/dL before meals and 11.8 mg/dL two hours after meals. The composition of staple foods remained relatively unchanged; therefore, portion control and further clinical evaluation are still needed. Integrating education, physical activity, digital monitoring, and community health worker support has the potential to strengthen independent diabetes management.
